Just went to 4.56 gears in my 94 Ranger 4x4 prerunner. So of course, the little shudder it used to have right as I took off is now a bigger shudder. And with the driveline revolving faster than before I now have a whrr-whrr-whrr at high speed. Is a CV at the trans and a fatter driveshaft the best fix? Thanks

I never had any issues with 4.56s. The whrr whrr at high speeds sounds more to me that your back lash is too tight.

Try this, remove the drive shaft from the diff plate, rotate 180 deg. and re-install. I had the same problem with my Jeep. This fixed it!
The shuddering is probably a pinion angle problem. Don't forget to lock-tite those drive shaft bolts!

Is it safe to assume it's been properly balanced and not seen and damage?

Since you built a prerunner with a drive line vibration, it is fair to ask a couple basic questions to qualify the obvious before offering advise. A little patience might have gotten you farther.

The driveshaft is stock and pristine. The center support was replaced a year ago. It's always had a little shudder on take off (an acceptable shudder), but now it's more pronounced. The whrring at speed is all new. The truck was built back in 1997 by Penhall Fabrication, and it's been great. I'll get around to rotating the driveline 180 degrees this weekend. I'll also see about balancing the shaft next week. Checking on the backlash might take a little longer. Again, thank you all for the help and I'll report back on what works.
